Comprehensive Systems Navigation provides care coordination to assist youth and families in navigating multiple systems, e.g., mental health, medical, disability services, etc. We help families secure resources for youth that are critical to their success, both now and into adulthood.
Often, our youth qualify for higher-level services from systems other than the Children’s Division, such as the Department of Mental Health, the Division of Developmental Disabilities, Social Security, etc. However, access is complicated, multi-layered, and fractured.

How Does It Work?
Any caregiver or professional can reach out for help. Comprehensive Systems Navigation cases routinely last one to two years. Navigators begin with an assessment of the child’s needs, including mental health, developmental delays, medical concerns, and placement stability.
Ultimately, however, the family is the expert; therefore, the goals and service plan are co-created among the caregivers, youth, and our navigator. Throughout the case, the family is our partner. Not only do our navigators secure services for the youth, they also teach caregivers how to overcome barriers on their own.
Once the service plan has been achieved, we regularly follow up with the family to ensure they have all necessary tools and information. It is the best feeling when a caregiver calls with excitement, saying they were able to navigate systems themselves!

What Makes it Unique?
Our Comprehensive Systems Navigators have a deep knowledge of local systems, funding streams, and effective services. They are expert advocates who know how to overcome barriers, including policy exceptions, access to waiver funding, and roadblocks inherent to each organization.
Who is Eligible?
Any child (ages 3-20) who:
- Is currently in foster care, or who has exited foster care to guardianship or adoption.
- Lives in St. Louis City or the counties of Franklin, Jefferson, Lincoln, St. Charles, St. Louis, or Warren.
Measuring Impact
Children maintain/improve on measures of well-being at case closure*
Children remain in their home or transition to a more stable living situation
*and Parent Efficacy Scale-Developmental Disability (CAPES-DD)
